Apply For Waiver of Premium Disability Benefits
You are eligible for benefits if you have been totally and continuously disabled for six months and your life insurance contains a Waiver of Premium Disability Benefits rider.
You can file a claim by completing forms that are available online under Printable Forms at the Service Center site. Standard forms include:
You and your doctor must complete the forms after six months of total disability. If you have been disabled for over 12 months, the maximum refund of premium you will receive is 12 months back to the date the home office receives written notice of your claim.
