If an individual has truly been named Guardian through a formal court appointment, the Guardian must sign all future-requested transactions. A copy of the formal court appointment must be submitted one time for coding on our records. Also, with any requested-transaction form being submitted by a Guardian, a Guardian Certification form* is required.
IMPORTANT NOTE: The courts require annuity and/or insurance contract transactions be handled differently than other assets. Court approval, most likely through the Probate Department of a local courthouse, is needed prior to any requested transaction being processed. A letter on the court’s letterhead approving the transaction must accompany the forms.
